O'Donnell and Phillips, payment is due at the time of service. Payment is
accepted by major credit card, debit card, check, cash, or Care Credit®. |

| We are
sensitive to the fact that some people may not be able to pay cash at
the time of treatment; therefore, we offer an extended monthly payment
plan for the convenience of our patients who qualify. Care Credit
Monthly Payment Plan is a separate line of credit, which does not
affect the balances of your other credit cards. Unlike other credit
cards, there are no annual fees, minimum monthly payments and it is
interest free. If you would like this option of payment, please ask your
dental provider for an application. Processing your application will
only take a few minutes. You can find out more about Care Credit at www.CareCredit.com

Our office participates with many different insurance plans and discount dental plans. We will bill your insurance company for services rendered. Please keep in mind that all co-pays are due at the time of service. We participate in the following plans. Please ask about your plan if you do not see it listed. Cancellation and No-show Policy Our office has a 48 -hour cancellation policy. If you are unable to keep your scheduled appointment, please call the office at (717) 293-2784. If calling after 5 pm, please leave a message on our voice mail. We reserve the right to charge a fee of $50 if you cancel with less than 48 hour notice or if you fail to show for an appointment. This fee can not be billed to your insurance. |
